For a new entrant seeking eligibility, the OUS 4 Worksheet must be completed and mailed to the Public Utility Commission of Oregon (PUC). A copy of the application and the service areas for which eligibility is requested are to be simultaneously mailed to the incumbent Telephone Company. For further information, see
