OUS Fund - Quarterly OUS 2 Revenue Reporting and Payment Schedule

The schedule for the quarterly OUS 2 revenue reports and contribution payments is shown below for recovery quarters through the end of the current year.  The reports and payments are due by the dates listed; reports not received by the due date are subject to a late form fee.  Payments not received by the due dates are overdue and subject to a late payment penalty and interest.  The timely filing of reports and payments is required to retain the telecommunication service provider certification.  Failure to file timely reports and payments may result in cancellation of certification in the State of Oregon.

Report forms will be mailed within two-four weeks of the report due date to the contact person reported on the OUS 1.  The forms may also be downloaded from the OUS Forms, Worksheets, Schedules, Instructions and FAQs.
