OUS Fund - Worksheet Summary |

Implementation of the OUS fund requires that all TSPs complete two types of OUS worksheets. Eligible LECs must complete four types of OUS worksheets. Details are provided in OUS Forms, Worksheets, Schedules, Instructions and FAQs. In summary, the worksheets are as follows: The OUS 1 identifies the TSP, the contact person, and the mailing address. It also certifies the accuracy of the information provided on the worksheets. All TSPs must file the OUS 1 Worksheet to establish the provider’s identification information on the administrator’s database. The provider must file a corrected worksheet whenever changes occur to its business, organization structure, mailing address, contact person, or certifying officer. In addition, the OUS administrator shall request updated provider identification information annually so that the current identification information on the administrator’s database is not older than 12 months. The OUS ID. number entered at the top of the worksheet is the Telecommunications Service Provider Company ID # found in eDockets. The OUS 2 Worksheet identifies quarterly telecommunications revenue. It categorizes revenue by wholesale, retail, and excluded. Oregon intrastate retail revenue is the assessment base for the OUS contributions. All certified TSPs operating in Oregon must file the OUS 2 Worksheets with the OUS administrator within 40 days of the close of each quarter (i.e., by the tenth day of the second month following the close of the quarter). The OUS 3 identifies basic telephone service access line-counts by wire center support area. The definition of basic telephone service is set forth in OAR 860-032-0260. Basic service line-counts provide the basis for calculating the size of the OUS fund and the monthly distribution of support. Only eligible LECs, designated by the PUC, must file the OUS 3 Worksheet. The OUS 3 Worksheet is due to the administrator within 40 days of the close of each month (i.e., by the tenth day of the second month following the close of the current month). LECs seeking eligibility must mail an application to the PUC. Only applicants having a current Oregon Certificate of Authority to provide local exchange switched service may apply. In addition to certification, the applicant must meet a number of criteria regarding the provision and advertising of basic telephone service before it can become eligible to receive OUS support.
